Scale mismatch
These units are marks on a U-100 barrel. Draw the same count on a U-40 and it carries 2.50× the mass, because one mark there is 0.025 mL rather than 0.01 mL. Check the barrel, not the box.

Barrel size sets how much fits in one draw, and nothing else. Mass per unit mark is 50 mcg on every one of them, because a 1 mL, 0.5 mL and 0.3 mL insulin syringe are the same U-100 scale at different capacities. Only changing the scale above changes what a mark is worth.
